I want to make a claim. What does MVAC cover?

What does MVAC not cover?

What medical benefits can I request?

My medicare and insurance only cover a portion of my medical costs. Can the medical benefits from MVAC pay for the rest of these expenses?

Do I have to sue in order for MVAC to become involved?

Who do I sue?

Are there time deadlines?

I am the victim of a hit-and-run driver. What do I do?

Judgment Payment Application Forms

Top

If a Judgment for personal injury cannot be paid by the uninsured party(ies) ordered to pay it, payment may be obtained through Motor Vehicle Accident Claims (MVAC) on completion of specific application forms. The forms contain information to satisfy the Administrator of the Motor Vehicle Accident Claims Act that all requirements of the Act have been met. They also assign the debt to the Adminstrator so that it can be recovered from the party ordered to pay in the Judgment. The Administrator's office provides the forms to the claimant or their lawyer or representative once it has been determined that MVAC will pay the Judgment.

The forms to apply for payment can be obtained directly from Motor Vehicle Accident Claims, or printed from the links below. (Note: Forms other than those provided by MVAC or printed from this website will not be accepted.)

When accessing the forms, be sure to download a copy of the checklist describing which forms are required for specific circumstances, and how to complete them.
