Used to claim past due arrears owed by an individual for a specified time period. This is required if you wish to register with MEP and the debtor had missed payments in the past. This ensures that we know exactly what time period the debtor has missed payments for so we can collect those payments on your behalf more efficiently. You are required to have this document sworn before a Commissioner for Oaths in and for the Province of Alberta.
