Fatality inquiry reports

The Minister of Justice may order that a Provincial Court judge conduct a public fatality inquiry. The inquiry will determine the identity of the deceased; the date, time and place of death; and the circumstances of death. Once the inquiry is complete the judge provides a written report.

The judge may make recommendations for the prevention of similar incidents, but can't make any findings of legal responsibility.

Report to the Attorney General - Public inquiry into the death of Shane Westrand 
December 06, 2006

Shane Westrand was arrested in Grande Prairie on September 10, 2002. He had been using drugs. Corrections staff at the Peace River Correctional Centre brought him to Grimshaw Hospital when he became lethargic and unwell. His condition continued to deteriorate and he was pronounced dead in Grimshaw Hospital on September 15, 2002.

November 30, 2006

On November 13, 2004, six-year-old M.A was ejected from the rear seat of a vehicle involved in a single vehicle rollover. The Child, Youth and Family Enhancement Act prohibits publishing the name of the child.

Report to the Attorney General - Public inquiry into the deaths of Corporal James Galloway and Martin Ostopovich 
November 21, 2006

Martin Ostopovich was involved in an armed standoff with police on February 28, 2004. He shot police officer James Galloway and was subsequently shot at by four members of the police tactical team. He was transported to Stony Plain Hospital where he was pronounced dead. Corporal Galloway was pronounced dead at the scene.

November 15, 2006

Ryan Silver and Ryan Eckhard were working at the Hub Oil recycling plant on August 9, 1999, when there was a series of large explosions and fires that demolished the plant.

Report to the Attorney General - Public inquiry into the death of Patricia Rivard 
October 18, 2006

Patricia Rivard was released from the Edmonton Institution for Women to a halfway house after completing two-thirds of her sentence. The cause of death was determined to be morphine toxicity. She died October 28, 2003.

Report to the Attorney General - Public inquiry into the death of Douglas Colp 
July 24, 2006

Douglas Colp was involved in a standoff with police on April 10, 2005. RCMP heard gunshots and found Colp unresponsive with a self-inflicted gunshot wound. He was transported to the University of Alberta Hospital where he was pronounced dead on April 11, 2005.

July 11, 2006

Christopher Lapatak was found unresponsive in his cell at the Edmonton Remand Centre on the morning of November 12, 2004. The cause of death was determined to be a drug overdose.

Report to the Attorney General - Public inquiry into the death of Deng Fermino (Kuol) 
May 30, 2006

Deng Fermino was involved in a disturbance at another person’s residence in Calgary, to which the police responded. An altercation ensued between the responding police officer and Mr. Fermino. Mr. Fermino injured the officer with a knife and was shot twice. He died October 25, 2003.

May 04, 2006

Connorregge Holloway was a pedestrian on Highway #1 West at the Chief Chiniki Overpass when he was struck by an automobile. Approximately 10 hours after the reported collision he was found beside the highway near the collision site. He died July 4, 2004.

May 02, 2006

Kyle James Young, a young offender, died January 22, 2004 at the Edmonton courthouse after he fell down an elevator shaft.

March 13, 2006

Colin Alexander Corr was found unresponsive and hanging by his neck in his cell on May 25, 2004.

Report to the Attorney General - Public inquiry into the death of Jody Nicole Umpherville 
February 13, 2006

Jody Umpherville had a history of intravenous drug abuse. She was arrested and admitted to the Edmonton Remand Centre August 8, 2003. She went into drug withdrawal and was given various medications to counteract it. CPR was initiated when she was observed to stop breathing on August 11, 2003, and paramedics arrived six minutes later. Despite advanced cardiac life support measures she was pronounced dead at the scene.

January 31, 2006

On September 3, 2004, while participating in a preseason high school football practice, Tyler Travis Zeer struck his head in a helmet to helmet hit with another football player. Tyler Zeer walked away from the drill but shortly thereafter began to show signs of having suffered a concussion. His condition deteriorated quickly. Tyler Zeer died in hospital on September 8, 2004.

Report to the Attorney General - Public inquiry into the death of Stella Jane (Jennie) Nelson 
January 24, 2006

Jennie Nelson, who had a history of multi-infarct dementia, atherosclerotic cardiovascular disease, hypertension, and chronic obstructive pulmonary disease, suffered second-degree burns to both legs when she was placed in a tub of hot water on January 2, 2004. She was transferred to the University of Alberta Hospital Burn Unit. Her condition deteriorated and her family decided on compassionate care measures only. She died on January 11, 2004.

January 23, 2006

Clifford Way was discovered suspended by his neck with a rope from a tree a short distance from Alberta Hospital Ponoka. He was found dead December 7, 2003.
