The Minister of Justice may order that a Provincial Court judge conduct a public fatality inquiry. The inquiry will determine the identity of the deceased; the date, time and place of death; and the circumstances of death. Once the inquiry is complete the judge provides a written report.
The judge may make recommendations for the prevention of similar incidents, but can't make any findings of legal responsibility.

On February 18, 2007, William Wilson was found unresponsive at the bottom of a stairway in a bar. He was transferred to hospital where he presented with alcohol intoxication and cranial trauma. His condition worsened and he died February 27, 2007.

On April 1, 2006, an intoxicated James Leonard Doyle was seen to fall backwards and strike his head. He was transported to hospital where he refused treatment, became physically aggressive, and was moved to police cells. He was later noted to have difficulty responding and was transported to hospital where his condition deteriorated and he died on April 14, 2006.

On March 11, 2005, Daniel Lippa was holding a knife and advancing on a police officer when he was shot.

On August 26, 2005, Irene Raffa became short of breath at her nursing home. EMS attended and while she was being transferred to a stretcher she fell and hit her head. She was admitted to hospital and died on August 30, 2005.

On April 4, 2005 Jeremy Paquette was engaged in underwater breath holding activities at a supervised public swimming pool where he was noted to be unresponsive. Resuscitation was unsuccessful and he died on April 8, 2005.

On March 19, 2004, Ronald Perry was arrested for strange and violent behaviour. In an effort to restrain him he was electronically stunned by police. He was transported to hospital where he suffered cardiac arrest and died on March 23, 2004.
