Trust Agreement

This is the document that you use to enroll the AISH recipient in the program. It is to be completed and signed by the Primary Contact Person and the AISH recipient. This form is required when enrolling an AISH recipient in the program.

AISH Benefits Administration Program Instruction Form/Budget   
Legal Sized (with seven budget lines) 
Letter Sized (with four budget lines) 

This form sets out the AISH recipient's instructions to the Office of the Public Trustee with respect to how their AISH benefits should be spent (i.e. it is the AISH recipient's budget.) This form is required when enrolling an AISH recipient in the program. See also: How to make changes to the Instruction Form.

This is an example of how the Instruction Form/Budget should be completed.

Contact Provider Co-ordinator Verification Form 

This is an attachment to the Trust Agreement and is to be signed by the Program Contact Person. This form is required when enrolling an AISH recipient in the program.

Acknowledgement Form 

This is an attachment to the Trust Agreement and is to be signed by the AISH Financial Benefits Worker or AISH Administrator. This form is required when enrolling an AISH recipient in the program.

Termination Form 

This is an attachment to the Trust Agreement. If the AISH recipient wishes to terminate their involvement in the program, this form must be signed by the AISH recipient and given to the Primary Contact Person.
