You must fill out a new Instruction Form/Budget and send it to the Office of the Public Trustee. Both you and your Primary Contact Person must sign the instruction sheet before you send it.

Every time you make a change to the way you want your AISH money to be used, you must fill out a new Instruction Form/Budget.

You cannot get money from the Public Trustee by going to their office without having the Instruction Form/Budget signed.
